Two QTc values can disagree even when the measured QT and heart rate are identical. Before calling the difference a physiological change, identify the correction formula. Comparing a Bazett value with a Fridericia value is not a like-for-like trend.
A Reproducible Example
Take a fictional adult sinus-rhythm tracing with measured QT 340 ms and heart rate 110 beats/minute. RR is 60 / 110 = 0.54545 seconds. This is an arithmetic teaching example, not an ECG interpretation or a patient-specific risk assessment.
Using QT in milliseconds and RR in seconds:
| Method | Expression | QTc, Rounded |
|---|---|---|
| Bazett | 340 / square root of RR | 460 ms |
| Fridericia | 340 / cube root of RR | 416 ms |
| Framingham | 340 + 154 x (1 - RR) | 410 ms |
| Hodges | 340 + 1.75 x (110 - 60) | 428 ms |
The spread is about 50 ms. Nothing about the underlying tracing changed during these calculations. Mixing seconds and milliseconds in the additive Framingham expression would introduce another, entirely avoidable error. The formulas and their unit conventions are described in the Penn sinus-tachycardia study.
What the Study Supports
That observational analysis included 6,723 people with sinus tachycardia and found that Bazett classified substantially more QT intervals as prolonged than the alternative corrections. It excluded baseline atrial fibrillation/flutter and a history of heart failure. It was not a trial showing that changing a formula improves outcomes or makes a particular medicine safe. Its conclusions should not be transferred automatically to children, irregular rhythms or every ECG measurement problem. Study methods and results.
Resolve Measurement and Context Before the Label
A formula comparison does not repair an incorrectly measured QT. Review the tracing, rhythm, QRS duration, identification of the end of the T wave and whether the measurement method is appropriate. Wide-QRS or otherwise difficult tracings need suitable clinical interpretation, not just another correction.
Use the QTc calculator to reproduce the arithmetic and record the selected formula. Its categorical colour or label is not a diagnosis of congenital long-QT syndrome, a torsades probability or permission to start or stop a medicine. Symptoms, medicines, electrolytes and the clinical setting require their own assessment.
For serial review, retain the measured QT, heart rate, rhythm and formula as well as the QTc. If the method changes, say so explicitly.
Question for handover: Yesterday's Bazett QTc was 460 ms and today's Fridericia QTc is 416 ms. Has repolarisation improved? The numbers alone cannot establish that. First compare the original measurements and methods. Selecting the lowest calculated value is not a safety strategy.
