How this calculator works
The ANC is derived from the CBC with differential using ANC (cells/microL) = total WBC (cells/microL) x (percent segmented neutrophils + percent band forms) / 100. It converts the relative neutrophil percentage into an absolute number, since a "normal" percentage on a very low or very high total WBC can mask clinically significant neutropenia or neutrophilia. Bands (immature neutrophils) are included because they retain antibacterial function; some laboratories report only segmented neutrophils, in which case bands must be added manually if a manual differential is available.
When to use this calculator
Use the ANC to grade neutropenia and stratify infection risk in patients on chemotherapy, with hematologic malignancy, aplastic anemia, or drug-induced agranulocytosis (e.g., clozapine, antithyroid drugs, sulfasalazine), and to define recovery after stem-cell transplant. It anchors the definition of febrile neutropenia and drives decisions about empiric antibiotics, growth-factor support, and isolation. Interpret it cautiously in populations with benign ethnic neutropenia (common in individuals of African, Middle Eastern, and Afro-Caribbean descent), where a baseline ANC of 1.0 to 1.5 x 10^9/L carries no increased infection risk, and in neonates, who follow age-specific reference ranges (the Manroe and Mouzinho charts) rather than adult cutoffs.

Worked example
A patient 10 days post-chemotherapy has WBC 1,200 cells/microL with a differential of 15% segmented neutrophils and 3% bands. ANC = 1,200 x (15 + 3)/100 = 1,200 x 0.18 = 216 cells/microL, i.e., 0.22 x 10^9/L. This is severe (profound) neutropenia (ANC < 500), placing the patient at high risk for serious bacterial and fungal infection. If this patient develops a single temperature of 38.3 C or sustained 38.0 C, they meet criteria for febrile neutropenia and warrant urgent empiric broad-spectrum antibiotics.
Limitations and safety notes
The ANC only reflects the reported differential: automated analyzers may not flag or quantify bands, and if only segmented neutrophils are counted the ANC can be underestimated in a left-shifted picture. A manual differential adds imprecision when few cells are counted, so at very low counts (< 200/microL) the reported ANC has wide uncertainty and trends matter more than single values. The count is a snapshot of the circulating pool and does not capture marginated or marrow reserves, so it can be transiently distorted by demargination (stress, exercise, epinephrine, corticosteroids) or margination (early sepsis, hemodialysis). Applying adult severity cutoffs to neonates or to patients with benign ethnic neutropenia overstates risk and can lead to unnecessary treatment interruption.
Frequently asked questions
Should band forms be included in the ANC?
Yes. Bands are functionally competent neutrophils, so the classic formula counts both segmented neutrophils and bands. Many modern automated analyzers do not separately report bands, so an ANC calculated from an automated count typically reflects segmented neutrophils only; add bands manually when a manual differential is performed and a left shift is present.
What ANC defines febrile neutropenia?
Per IDSA and NCCN, febrile neutropenia is an ANC below 500 cells/microL, or below 1,000 with a predicted fall to under 500 within 48 hours, together with a single oral temperature of 38.3 C or a temperature of 38.0 C sustained over one hour. It requires urgent empiric broad-spectrum antibiotics.
Why calculate ANC instead of using the neutrophil percentage?
The percentage is relative to the total WBC and can be misleading. A neutrophil percentage of 40% looks reassuring but corresponds to an ANC of only 400 if the total WBC is 1,000. The absolute number is what determines infection risk.
How does benign ethnic neutropenia change interpretation?
Benign ethnic (constitutional) neutropenia, associated with the Duffy-null genotype, produces a stable baseline ANC around 1.0 to 1.5 x 10^9/L without increased infection risk. Standard neutropenia thresholds and drug-monitoring rules (e.g., for clozapine) are adjusted downward for these patients to avoid unnecessary treatment interruption.
