Alvarado Score Calculator for Gastroenterology. Scores are commonly grouped into three bands: 1-4 makes appendicitis unlikely and favors discharge or observation without routine imaging; 5-6 is equivocal and warrants observation and cross-sectional imaging (or serial reassessment); and 7-10 signals high probability, supporting surgical consultation. How this calculator works

The Alvarado Score is an additive 10-point clinical prediction rule for acute appendicitis built from eight weighted findings, captured by the mnemonic MANTRELS. Three symptoms (Migration of pain to the right lower quadrant, Anorexia, Nausea/vomiting) and two of the signs (Rebound tenderness, elevated Temperature 37.3 C or higher) each contribute 1 point; right-lower-quadrant Tenderness and Leukocytosis (WBC over 10,000/microL) each contribute 2 points; and a neutrophil left Shift (over 75% neutrophils) contributes 1 point. The points are summed to yield a total from 0 to 10, with higher totals reflecting greater probability of appendicitis.

When to use this calculator

Use it to risk-stratify adolescents and adults presenting to the emergency department or acute surgical unit with acute right-sided or migratory abdominal pain suspicious for appendicitis, chiefly to decide who is low-risk enough for discharge versus who needs imaging or surgical review. It is a rule-out and triage aid, not a stand-alone rule-in for the operating room. It performs poorly and should not be relied upon in young children, pregnant patients, elderly patients with blunted inflammatory responses, or patients already on antibiotics or steroids, and it is not designed to distinguish complicated from uncomplicated appendicitis.

Worked example

A 24-year-old man reports pain that began periumbilically and migrated to the right lower quadrant (1) with anorexia (1) and one episode of vomiting (1). On exam he has RLQ tenderness (2), rebound tenderness (1), and a temperature of 37.6 C (1); WBC is 13,200/microL (2) with 82% neutrophils (1). Total = 9, placing him in the high-probability band, which supports proceeding to surgical consultation with focused imaging rather than prolonged observation.

Limitations and safety notes

Pooled data show the Alvarado score is more useful for ruling out than ruling in disease: a 2018 meta-analysis of 2,161 patients found sensitivity around 69% and specificity around 77%, so a substantial minority of true appendicitis is missed and many high scorers do not have the disease. It systematically overpredicts in women of reproductive age (gynecologic mimics inflate tenderness and leukocytosis) and underperforms in children under about 5 and in immunosuppressed or elderly patients whose fever and white-cell response are muted. Because leukocytosis and left shift supply 3 of the 10 points, prior antibiotics, corticosteroids, or a resolving early presentation can falsely lower the total.

What is the mnemonic for the Alvarado Score components?

Why is a high Alvarado score not enough to go straight to appendectomy?

The score's specificity is only moderate (about 77%), so operating on a high score alone yields an unacceptable rate of negative appendectomies. Current WSES guidance is to use the score to select patients for confirmatory imaging (CT in adults, ultrasound first in children and pregnancy) rather than as a direct indication for surgery.

How does the Alvarado score differ from the RIPASA and AIR scores?

RIPASA is more sensitive (around 94%) but far less specific, making it better for ruling out and worse for confirming. The Appendicitis Inflammatory Response (AIR) score adds graded CRP and is often preferred in children. The Alvarado score remains popular because it needs only history, exam, and a white-cell count with differential.

Can the Alvarado score be used in children?

It can risk-stratify older children but is unreliable below roughly 5 years of age, where presentations are atypical and perforation risk is high. Many pediatric services prefer the Pediatric Appendicitis Score or AIR score, and a low Alvarado score in a young child should not override clinical concern.

What is the modified Alvarado score?

The modified Alvarado score omits the neutrophil left-shift item, reducing the maximum to 9 points. It is used where a manual differential is unavailable, but it drops one of the inflammatory markers and so is generally considered slightly less discriminating than the full 10-point version.
