ICH Score Calculator
ICH Score Calculator for Neurology. A severity total is not a personal mortality prediction. AHA/ASA guidance states that severity scores must not be the sole basis for limiting life-sustaining treatment: https://www.ahajournals.org/doi/10.1161/STR.0000000000000407

How this calculator works
Sum initial GCS (0-2), hematoma volume >=30 mL (1), IVH (1), infratentorial origin (1), and age >=80 (1). Original maximum is 6: https://pubmed.ncbi.nlm.nih.gov/11283388/
When to use this calculator
Communicate severity of adult spontaneous intracerebral hemorrhage. It does not apply unchanged to traumatic bleeding or replace a reliable initial examination.
Inputs used
- Adult spontaneous ICH with assessable initial data
- Age in years
- GCS integer 3-15
- Intracerebral hematoma volume in mL
- Intraventricular extension
- Infratentorial hemorrhage origin
Clinical interpretation
A severity total is not a personal mortality prediction. AHA/ASA guidance states that severity scores must not be the sole basis for limiting life-sustaining treatment: https://www.ahajournals.org/doi/10.1161/STR.0000000000000407
Worked example
Age 80, GCS 12, volume exactly 30 mL, IVH present and supratentorial origin gives 1 + 1 + 1 + 1 + 0 = 4. A volume of 29.9 mL gives no volume point.
Limitations and safety notes
Sedation can obscure GCS; volume estimation can be inaccurate for irregular hematomas. The widget does not calculate volume from images or encode preferences, reversible problems or treatment response.
Frequently asked questions
Are mL and cm3 different volume inputs?
They are numerically identical. Do not enter a diameter in place of hematoma volume.
