How this calculator works

MESS adds skeletal/soft-tissue points, ischemia points, shock points and age points. Age below 30 contributes zero, 30-50 contributes one and above 50 contributes two. Only the ischemia contribution doubles when duration exceeds six hours.

When to use this calculator

Use the score as structured documentation in adult lower-extremity trauma with threatened perfusion. Record the timing of ischemia and resuscitation explicitly. Immediate assessment and restoration of perfusion take priority over completing a score.

Worked example

At age 40, injury category 3, ischemia category 2 and transient shock 1 give 3 + 2 + 1 + 1 = 7 at exactly six hours. At 6.1 hours, the ischemia contribution becomes four, giving 9, not 14.

Limitations and safety notes

This form is restricted to adults and the published threatened-limb categories, not normal perfusion or upper-extremity validation. Ischemia duration may be uncertain. The score cannot represent patient preferences, reconstructive options or future functional recovery.
