How this calculator works
The 2009 harmonized definition counts five domains: waist, triglycerides, HDL, blood pressure and fasting glucose. Any three meet the definition. The tool evaluates each domain explicitly, including source-specific medication alternatives; nothing defaults to absent because a box was left unchecked.
When to use this calculator
Use when documenting the harmonized adult criteria. Specify the applicable waist cutoff rather than assuming one threshold fits every population. All numeric values are required in this implementation even when treatment qualifies a domain; do not invent missing measurements.

- Waist measurement and a clinician-selected population-appropriate waist cutoff, in cm or inches
- Fasting triglycerides, HDL and plasma glucose in mmol/L or mg/dL
- Systolic and diastolic pressure in mmHg
- Separate explicit treatment status for triglycerides, low HDL, hypertension and hyperglycemia

Worked example
A male assessment with waist 100 cm and selected cutoff 102, triglycerides 150 mg/dL, HDL 39 mg/dL, pressure 130/80 and glucose 99 mg/dL, with no qualifying treatment, meets three domains: triglycerides, HDL and blood pressure. Waist and glucose do not qualify.
Limitations and safety notes
This is not the central-obesity-required IDF definition used by NAFLD-LFS, and not pediatric criteria. SI calculations use the converted mg/dL boundaries rather than rounded SI equivalents, so very close boundary values require attention to the actual report. General statin use does not automatically establish both lipid-treatment indicators.
Frequently asked questions
Must waist be elevated?
Not in the harmonized definition. Three other qualifying domains can meet it.
Are two domains reassuring?
Not necessarily. Each abnormality merits its own assessment regardless of the total.
