How this calculator works

The cleanest approach converts testosterone from ng/dL to pg/mL (multiply by 10) so both hormones share pg/mL, then divides T by E2; some labs instead express both in pmol/L. Because of the unit mismatch, a raw ratio is meaningless unless the tool states its convention, so always read off the displayed units. The output is a single unitless index summarizing the balance between the primary androgen and its aromatized estrogen product.

Limitations and safety notes
The ratio has never been prospectively validated as a diagnostic threshold, and reference ranges vary widely between publications because of differing units and assays. Estradiol in men is low and must be measured by a sensitive liquid chromatography-tandem mass spectrometry assay; standard immunoassays overestimate low male estradiol and can distort the ratio badly. The metric is meaningless in women, children, and pubertal boys, and it does not account for SHBG or free hormone fractions, so two men with identical ratios can have very different bioavailable hormone status.
