Lung Cancer Risk Calculator
Lung Cancer Risk Calculator for Oncology. Meeting the rule supports a screening discussion; it is not a scan order. Failing the rule is not proof of low cancer risk. Screening can lead to false-positive findings, further procedures or detection of disease that would never have caused harm; those possibilities belong in the decision.
For shared guidance on choosing a tool and documenting results, read the clinical calculator workflow.
How this calculator works
This is a screening eligibility rule, not a cancer-risk model. It checks age 50-80, at least 20 cigarette pack-years and current smoking or fewer than 15 smoke-free years. The exact 15-year boundary follows the source's explicit instruction to discontinue at 15 years. Symptoms and health/surgery stopping conditions take priority.
When to use this calculator
During shared decision-making about annual low-dose CT for an otherwise appropriate screening candidate. Symptoms that could reflect cancer require diagnostic assessment rather than this preventive screening workflow.
Inputs used
- Current age in years, used against the USPSTF 2021 screening age limits
- Cumulative cigarette pack-years, summed across differing smoking periods
- Current, former or never cigarette smoking; elapsed quit years only for former smokers
- Symptoms potentially needing diagnostic evaluation rather than screening
- Health-related limits on life expectancy or surgery and willingness for curative lung surgery
Clinical interpretation
Meeting the rule supports a screening discussion; it is not a scan order. Failing the rule is not proof of low cancer risk. Screening can lead to false-positive findings, further procedures or detection of disease that would never have caused harm; those possibilities belong in the decision.
Worked example
A fictional 60-year-old who smoked 10 cigarettes daily for 40 years has (10 / 20) x 40 = 20 pack-years. If asymptomatic, ten years after quitting, without a health/surgery limitation and willing to consider curative surgery, the criteria are met. At fifteen years after quitting the stopping instruction applies.
Limitations and safety notes
No PLCOm2012, nodule malignancy or personal cancer probability is calculated. The catalog name is retained for parent integration, but the component explicitly names the narrower workflow. Other guidelines and coverage rules can differ. Pack-years concern cigarettes, not vaping or passive exposure. Input bounds are error checks, not validated biological limits.
